Yes, there are several people I know of who have bought airline tickets along with a car rental, supposedly including all required insurance, then told when going to pick up the car that they need to buy additional insurance. I don't know what company it was, but happened at the PV airport, both to friends of my neighbor and one of my Airbnb guests.
There is a privately owned car rental company in my area that everyone has good experiences with, whose contact info I now give to any guest who intends to rent a car, warning them of these scams.

Yes, the airport rental companies are horrible for this. Happened to me about 15 years ago when arriving at GDL. The rental was $18 USD per day when I booked it. They wanted another $35 USD per day for insurance when I arrived to pick it up. I politely told them to stuff it and rented a local beater from Mark Turford who was about the only local game in town at the time.
