I have misplaced my copy of the original vaccination form. Does anyone have the website so I can reprint it?

I just did this again although, when you fill in your Curp Number, a notice comes up that you've already received it. However, I DID receive it again via e-mail. YOU HAVE ONLY 90 MINUTES AFTER RECEIVING THE E-MAIL TO CLICK ON IT. So keep checking your e-mail and spam folder until you receive. After clicking on the link in the e-mail, the pdf pops up and you can print it again. I would then save the pdf to your desktop (or wherever you choose) and also print off multiple copies.

Can't answer that question yet 'cuz I just got my third shot last Wednesday. However, it took two months to get the certificate for the first two shots. Paciencia, estamos viviendo en Mexico.

Thank you, however I'm looking for the original form with your name and CURP on it you had to present when getting the vaccination In Spanish its Represion del registeo para la vacuncion.

You won't be able to print off an original that hasn't been filled in with your vaccination info. If you want one that isn't filled in for the booster shot you can get it here.
