And under what circumstances is it appropriate/acceptable to tutear?

Tutear is addressing someone informally as tu instead of Usted. It means a more close relationship between the parties as for respect for professionals, people in a position of power or older people or those you just met you would use Usted and conjugate all verbs that way.

Each country is different, if in doubt, use Usted and be formal to not offend someone.

not only every country, I have noticed that people are much quicker to say tu in Guadalajara than in Chiapas.

In Colombia, for example people with close relationships still use Usted and then there is always vos.

In Frence tu is more comon in the south than in the north and vous seems to be disappearing for the young. It is also a class thing , the upper conservative class tend to havepeople within the same family not use the tu when in the Basque country you can still hear parents say tu to the children and children say vous to the parents.

I recently was speaking with a well bred young Tapatio who was telling me he was having problem saying usted to anyone but his parents insisted that he did,

The tu and usted usage does not have a strict rule and each class, geographic area has its nuaces has a lot of nuances , I would guess in any country that has the two forms. As a foreigner you are better off using the Usted with adults until they tell you not to .

In Costa Rica tu is never used,just usted and occasionally vos.
